Community Wellness Network

The Community Wellness Network brings the entire Medical Home program to the clinic  population through the internet

 

• Notifies patients of appointments and activities in the clinic

 

• Allows for the patient to have direct communication with the clinic

• Allows for the patient to review their chart, schedule appointments and review their account

​

• Allows the patient to be active in the Health and Wellness Resource Center remotely

​

• When traveling allows the patient to remain in contact with the complete Medical Home; Clinical Wired for Health and Health and Wellness Center

 

• When needed the patient may utilize the Wired for Health program

 

• The Community Wellness Network makes the clinic the Medical Home of the community whether in the community or outside of it.

• Social Center for the Medical Home of the Clinical Community

​

• Network for programming ( all programming provided via the internet to ensure Quality Control of programming to all clinics)
